Join a respected organization as a Home Health Nurse (RN), where you will provide vital home care services to seniors and community members, helping them maintain independence and safety in their own homes. This role offers a balanced schedule, Monday to Friday from 9:00 AM to 5:00 PM, with onsite training and flexible scheduling afterward. The position provides an estimated annual pay range of $90,000 to $94,000.

Responsibilities
- Visit patients at home to assess health, safety, and daily living needs
- Determine eligible services, such as home health aides or therapy programs
- Coordinate care plans with healthcare providers and community organizations
- Communicate effectively with families regarding care plans and requirements
- Maintain detailed records of assessments and interventions
Credentials
- Valid RN license in Massachusetts
- Associate degree or higher in Nursing
Required Experience and Skills
- Minimum of 1 year of nursing experience
- Community nursing experience preferred
- Interest in working with elderly populations
- Strong organization, communication, and clinical judgment skills
- Ability to work independently and coordinate care effectively
